Medical Social Worker

Our Medical social workers are patient advocates and counselors. They perform social service assessments, refer patients and families to medical resources, and provide patient and family assistance in obtaining community resources.  Most importantly, a medical social worker works to assure that the best interests of the patient are being met consistently.

Counseling

Our medical social workers counsel patients and their families. They understand and explain the nature of illness and advice patients and families on how to effectively address the symptoms and treatment. Our medical social workers also serve as grief counselor to help patients and families deal with the trauma of experiencing chronic or acute illness.

Care Planning

Generally, families and patients have very little idea about where to turn for medical help. The time it takes for the families to set out on the right path has been known to be long and error prone. This is where our medical social workers can be your guiding light. Our medical social workers, after a careful evaluation and assessment, assist patients and families in finding and arranging services such as in-home care, nursing home care, and counseling.

Financial Assistance

Chronic illnesses are expensive. A large percentage of families is not in a position to afford the cost of treatment. Imagine if the patient is a parent, the financial support for managing the dependents also needs to be considered. Our medical social workers refer and assist patients in obtaining financial assistance, food assistance, and health care coverage through city, state, and federal programs.

Advocacy

Our medical social workers, in times of need, are also patient advocates. Acting as an intermediary between the patients and the medical community, our medical social workers act as a necessary bridge: a voice for those who have communication barriers or cultural differences, enabling effective communication, and empowering those who need empowering.
